These Terms & Conditions (“Terms”) govern your use of Sereniva’s website, telehealth services, and digital platforms. By accessing our site or using our services, you agree to these Terms and our Privacy Policy.
Eligibility for Services
You must be at least 18 years old to use Sereniva’s services. By scheduling an appointment or creating an account, you confirm you meet this requirement and that all information provided is accurate.
Nature of Services
Sereniva provides telehealth consultations and wellness services led by licensed, board-certified Nurse Practitioners. We do not provide emergency care. In case of an emergency, call 911 or visit the nearest hospital.
Cash-Pay Model
Sereniva operates on a self-pay (cash-pay) basis. We do not accept insurance for consultations or ongoing care. However, laband prescriptions may be submitted to third-party facilities that accept insurance.
Payment & Fees
Payment is due at the time of service. A fee schedule will be provided in advance via email. By booking an appointment, you agree to pay all applicable fees. Fees are non-refundable unless explicitly stated in writing.
Cancellations & No-Shows
We require at least 24 hours’ notice for cancellations or rescheduling. Late cancellations or missed appointments may result in a fee. Repeated no-shows may lead to account suspension.
Consent to Telehealth
By using Sereniva’s services, you consent to receive healthcare via telecommunication technologies. You acknowledge the limitations of virtual care. A separate Telehealth Consent Form will be provided and must be signed before your first visit.
Medical Records & Communication
Sereniva uses HIPAA-compliant systems to manage and store your health information. You are responsible for maintaining the confidentiality of your login credentials. Communications may occur via secure portal, email, or messaging.
Intellectual Property
All content on our website, including logos, text, graphics, and software, is the property of Sereniva or its licensors. You may not reproduce, distribute, or modify content without prior written consent.
Modifications to Terms
We may revise these Terms at any time. Continued use of our services indicates your acceptance of any updated Terms. The current version will always be posted on our website.
Termination of Access
We reserve the right to terminate or restrict access to our services at our discretion, particularly in cases of abuse, fraud, or policy violations.
Limitation of Liability
Sereniva is not liable for any indirect, incidental, or consequential damages arising from your use of our site or services, including care delays or technical issues.
Dispute Resolution
All disputes arising under these Terms shall be governed by the laws of the State of New Jersey and resolved through binding arbitration unless otherwise required by applicable law.
